All forms are required before participation:   Your Team Mom will provide these forms
1. registration form- online submission
2. medical release VHS school physical (dated after May 2012) or may use AYF clearance dated January 2012.
3. report card (2011-2012 with final grades)
4. waiver form
5. parent code of conduct
6. AYF consent to play
7. AYF consent to treat
8. any valid passport, dmv id, or military id
9. copy of a birth certificate
10. Concussion form
 

Where does my registration money go?
*insurance (player and board members)
*practice and game fields (linings/mowings)
*referees
*administration (paper work)
*Prince William County per play fee
*equipment maintenance/storage
*accounting/audit/tax services
*AYF membership dues
*VAYFA membership dues
Unfortunately, registration fees does not cover special events, banquets, trophies, concessions, team travel etc....therefore, parents are expected to participate in fundraisers and or make donations toward their teams expenses. All donations are tax deductible.

NYVAA is a subordinate organization of American Youth Football, Inc, an NFL Youth Partner. NVYAA is a partnership with Hylton Boys & Girls Club. I understand that before my child will be placed on a team, the total registration fee is required. I understand equipment/uniforms are to be returned at the last game of the season or pay a $250 fine. I also confirm and acknowledge my child is physically healthy and I grant my child permission to participate in any/all activities associated with the NVYAA, Hylton BGC, VAYFA and AYF. I agree to assume any/all risks and hazards incidental to participation including transportation to and from all activities. I agree to hold harmless and release from liability the NVYAA, Hylton BGC, VAYFA, & AYF, and their respective officers, directors, sponsors, employees or volunteers for any injuries sustained by my child as a result of participation in any of the aforementioned activities. As a parent, I agree to uphold all statements in the Parent Code of Conduct and will conduct myself appropriate at all times. I understand that I must submit all monies/sponsorships/finances to an Executive Board member only for accountability. I understand the coach or team mom is not an Executive Board Member in most cases. Submitting any paperwork or any participation in any NVYAA/Hylton BGC/VAYFA/AYF means I agree to all of the above terms and NVYAA/VAYFA/AYF by-laws.
