NYVAA is a subordinate organization of American Youth Football, Inc, an NFL Youth
Partner. NVYAA is a partnership with Hylton Boys & Girls Club-Cardinals, Cowboys & Bengals. I understand that before
my child will be placed on a team, the total registration fee is required. I also confirm and acknowledge my child is physically
healthy and I grant my child permission to participate in any and all activities associated with the NVYAA, Hylton BGC, VAYFA
and AYF. I agree to assume any & all risks and hazards incidental to participation including transportation to and from
all activities. I agree to hold harmless and release from liability the NVYAA, Hylton BGC, VAYFA, & AYF, and their respective
officers, directors, sponsors, employees or volunteers for any injuries sustained by my child as a result of participation
in any of the aforementioned activities. As a parent, I agree to uphold all statements in the Parent Code of Conduct and will
conduct myself appropriate at all times. I understand that I must submit all monies/sponsorships/finances to an Executive
Board member only for accountability. I understand the coach or team mom is not an Executive Board Member in
most cases. Submitting any paperwork or any participation in any NVYAA/Hylton BGC/VAYFA/AYF means I agreed to all of
the above terms and NVYAA/VAYFA/AYF by-laws.
